CHALLENGE
A 67-year-old Caucasian male in rural Tulare County reports full medication adherence, yet his HbA1c remains above goal. On the surface, everything appears managed until a pharmacist trained in assessing social determinants of health (SDOH) by aPHP identifies deeper challenges: unaddressed mental health needs and limited digital literacy, both preventing consistent access to follow-up care.
